A timelydiagnosis is vital due to the serious complications thatcan occur, including heart failure, thromboembolism,or tachyarrhythmias. We report the case of a 78-yearold male with history of previous anterior myocardialinfarction and currently under evaluation by chronic heartfailure. Transthoracic echocardiogram rev­ealed a hugethrombosed and calcified anteroapical left ventricularaneurysm. Coronary angiography demonstrated that theleft anterior descending artery was chronically occluded,and revealed a big and spherical mass with calcifiedborders in the left hemithorax. Left ventriculogramconfirmed that this spherical mass was a giant calcifiedleft ventricular aneurysm, causing very severe leftventricular systolic dysfunction. The patient underwentcardioverter-defibrillator implantation for primaryprevention. %~
