After closer observation, echocardiography showed a large pseudoaneurysm in the posterior mitral annulus opening into the left ventricle. Anatomical assessment by 2D and 3D transesophageal echocardiogram (TEE) was essential for a detailed morphologic characterization and surgical procedure delineation.
