    Time–efficiency assessment of guided toric IOL cataract surgery: a pilot study

    Purpose: To compare the time spent on toric intraocular lens (IOL) implantation during cataract surgery using a manualmarking vs a digital image–guided system (the Verion) for toric IOL alignment. Settings: All procedures were performed at the Instituto Oftalmológico Quirónsalud ophthalmology clinic (A Coruña, Spain). Design: We designed an experimental and longitudinal (1-month follow-up) study. Methods: A total of 98 eyes of 65 participants (68.2 ± 12.2 years) were divided into 2 groups: 49 eyes operated with toric IOL alignment using a manual-marking technique (manual group) and another 49 eyes operated using image-guided marking (Verion group). The primary variable for comparison between both groups was cataract surgery time. Other outcomes such as toric IOL misalignment, spherical equivalent, astigmatism, uncorrected distance visual acuity, and corrected distance visual acuity were also measured. AU3 Results: The total cataract surgery time was 2:09 minutes shorter (P < .001) with the Verion system (12:12 ± 2:20) compared with the surgical procedure performed using manual marking (15:27 ± 3:04). One month after surgery, there were no statistical differences in terms of toric IOL misalignment between the Verion (3.38° ± 2.95°) and the manual group (4.66° ± 3.95°). No statistical differenceswere observed between groups for refractive and visual outcomes either (P ≥ .05). Conclusions: The cataract surgery time was reduced when the procedure was assisted using the Verion system to align the IOL compared with manual marking, maintaining the same efficacy in terms of toric IOL misalignment, residual refraction, and visual acuity.

    The influence of surgeons and technicians on the learning curve of femtosecond-laser cataract surgery

    Purpose: To demonstrate the influence of the surgeon's and the operating room (OR) technicians' experience upon the outcome of femtosecond laser-assisted cataract surgery (FLACS). Materials and methods: Our study included 250 eyes from 156 patients who had undergone either cataract surgery or clear-crystalline-lens extraction and where capsulorhexis and lens fragmentation had been performed using the CATALYS® Precision System femtosecond platform (Abbott Medical Optics Inc., Santa Ana, CA, USA). The patients were operated either by an experienced surgeon in the use of femtosecond laser or by an inexperienced surgeon in that field and two technicians. The quantitative outcome measures were: Suction loss rate, vacuum time, number of consumables used by the patient, and intraoperative complication rate. Results: Both for the experienced and the inexperienced surgeons, suction loss rates as well as vacuum time decreased progressively as time went by and more surgical procedures had been completed by that surgeon. For a given surgeon suction time decreased significantly, going from 137 to 99s, as the assisting technician gradually gained experience. The number of consumables used in each procedure by the experienced surgeon ranged from 1.10 (for the first 50 cases) to 1.02 from those initial cases onwards. Regarding intraoperative complications, they also decreased progressively as the number of procedures completed by the surgeon increased. Conclusions: The experience of each team member involved in such procedures-be it surgeons or technicians-have an impact, to a greater or lesser extent, upon the surgery's outcome, as quantified by the outcome variables of choice.

    Agreement and clinical comparison between a new swept-source optical coherence tomography-based optical biometer and an optical low-coherence reflectometry biometer

    Purpose To compare measurements taken using a swept-source optical coherence tomography-based optical biometer (IOLmaster 700) and an optical low-coherence reflectometry biometer (Lenstar 900), and to determine the clinical impacts of differences in their measurements on intraocular lens (IOL) power predictions. Methods Eighty eyes of 80 patients scheduled to undergo cataract surgery were examined with both biometers. The measurements made using each device were axial length (AL), central corneal thickness (CCT), aqueous depth (AQD), lens thickness (LT), mean keratometry (MK), white-to-white distance (WTW), and pupil diameter (PD). Holladay 2 and SRK/T formulas were used to calculate IOL power. Differences in measurement between the two biometers were determined using the paired t-test. Agreement was assessed through intraclass correlation coefficients (ICC) and Bland-Altman plots. Results Mean patient age was 76.3±6.8 years (range 59-89). Using the Lenstar, AL and PD could not be measured in 12.5 and 5.25% of eyes, respectively, while IOLMaster 700 took all measurements in all eyes. The variables CCT, AQD, LT, and MK varied significantly between the two biometers. According to ICCs, correlation between measurements made with both devices was excellent except for WTW and PD. Using the SRK/T formula, IOL power prediction based on the data from the two devices were statistically different, but differences were not clinically significant. Conclusions No clinically relevant differences were detected between the biometers in terms of their measurements and IOL power predictions. Using the IOLMaster 700, it was easier to obtain biometric measurements in eyes with less transparent ocular media or longer AL.

    Biomechanical Stability of Three Intraocular Lenses With Different Haptic Designs: In Silico and In Vivo Evaluation

    Purpose: To assess the biomechanical stability of three different marketed intraocular lenses (IOLs) with different haptic designs (four-loop IOL [Micro F FineVision model] and double C-loop IOL [POD F and POD FT models], all manufactured by PhysIOL), in silico (computer simulation) and in vivo (in the context of lens surgery). Methods: An in silico simulation investigation was performed using finite element modeling (FEM) software to reproduce the compression test defined by the International Organization for Standardization and in vivo implantation in patients in the context of lens surgery was evaluated 1 day and 3 months postoperatively. IOL decentration and rotation were tested. In addition, the stress and strains were analyzed with the finite element method. Results: In the in silico evaluation, the compression force for the POD F IOL was slightly lower than for the POD FT IOL and Micro F IOL for all compression diameters. The axial displacement was maximum for the POD FT IOL and the tilt, rotation, and lateral decentration were substantially lower than the acceptable tolerance limits established in ISO 11979-2. In the in vivo evaluation, a total of 45 eyes from 45 patients were selected, 15 eyes for each IOL model under assessment. Statistically significant differences were found between the Micro F and POD F IOLs for lateral decentration in the x-direction (in absolute value) at 3 months postoperatively (P = .03). Conclusions: Although statistically significant differences have been found when comparing the displacement, tilt, and rotation between the different lenses, these differences cannot be considered clinically relevant, which would suggest that all three IOL models yield excellent stability in those terms.     Comparison of visual performance between two aspheric monofocal intraocular lens models

    Clinical relevance: It is important to distinguish between visual acuity, optical quality and quality of vision when outcomes obtained with intraocular lenses are evaluated. These parameters, that include objective and subjective tests, should be assessed to obtain results that is not biased. Background: To assess the difference in visual and optical quality between two monofocal intraocular lens models. Methods: : This was a prospective, parallel and randomised clinical study conducted at Miranza IOA, a private clinic in Madrid, Spain. Sixty patients were implanted bilaterally, 30 per group, with two aspheric IOLs with induced spherical aberration of −0.27 μm for Group A and −0.20 μm for Group B. Visual outcomes obtained at 1 and 3 months after surgery included both uncorrected (UCVA) and corrected monocular distance visual acuity (DCVA), objective scattering index (OSI), modulation transfer function (MTF) cut-off, Strehl Ratio (SR), contrast sensitivity defocus curve (CSDC), intraocular lens spherical aberration (SA), and longitudinal chromatic aberration of the eye. Activity limitations in daily life was assessed using CatQuest-9SF questionnaire. Results: There were statistically significant differences for DCVA (004 LogMAR; p = .008) and SR (0.03;p = .003) between groups. Outcomes related to CSDC showed statistically significant differences for vergences between −0.50 D and +1.00 D (3mmpupil) and for vergences of 0.00 D and +0.50 D (4.5 mm pupil) between groups. Overall, Group A showed better results regarding visual and optical quality, including a lower longitudinal chromatic aberration result in comparison to Group B. Patient satisfaction evaluated with CatQuest-9SF showed that Group A achieved better outcomes, although the differences were statistically significant only for the ‘Reading text on television’ item (p = 0.027). Conclusions: Both intraocular lens models showed excellent quantity of vision, optical and visual quality as well as high patient satisfaction. Despite this, the the Group A model provided slightly better outcomes than the Group B model.

    Optical Behavior of an Enhanced Monofocal Intraocular Lens Compared with a Standard One

    The aim of this work was to compare an enhanced monofocal (RayOne EMV RAO200E, Rayner) and standard monofocal (RayOne RAO600C Aspheric, Rayner) intraocular lenses (IOLs) for three nominal powers (+10.00 D, +20.00 D and +30.00 D) as a function of the optical aperture diameter (pupil diameter) using a commercial Schlieren phase-shifting deflectometer NIMO TR1504 (Lambda-X, Belgium). From the wavefront maps measured by this instrument, the radial power profiles, the spherical aberration coefficients of the Zernike polynomial expansion (as a function of the optical aperture radius), and the root-mean-square (RMS) of the high-order aberrations (HOAs) were obtained and analyzed by comparing the two models. The results showed that the effective added power that could be obtained with the enhanced model depended directly on the pupil size and the power of the IOL implanted. The higher additions were achieved with the higher nominal IOL powers. The relationship between the pupil diameter, the corneal aberration of the patients and the power profile of these IOLs could have a crucial implication on the far distance and the final effective addition. However, it is important to note that these findings should be clinically validated through the implantation of these models in patients’ lenses.

    Comparison of 3-month visual outcomes of a spherical and a toric trifocal intraocular lens

    Purpose: To evaluate visual outcomes and satisfaction after implantation of 2 trifocal intraocular lenses (IOLs): a spherical IOL and a toric IOL. Setting: IOA Madrid Innova Ocular, Madrid, Spain. Design: Prospective, controlled clinical trial. Methods: Patients (>50 years) were implanted bilaterally with either a trifocal spherical hydrophilic IOL (FineVision POD F) if corneal astigmatism was 1.0 diopter (D) or less, or with a trifocal toric hydrophilic IOL (FineVision POD FT) if astigmatism was more than 1.0 D. Outcomes analyzed 3 months after surgery included monocular and binocular visual acuities at distance, near, and intermediate, both uncorrected and corrected. Defocus curves, contrast sensitivity, and patient satisfaction were also assessed. Results: There was no statistically significant difference between groups in monocular uncorrected distance (UDVA) (P = .38), monocular corrected distance (CDVA) (P = .22), or distance-corrected intermediate (DCIVA) (P = .95) visual acuities; however, the distance-corrected near visual acuity (DCNVA) was slightly better in the spherical IOL group (P = .008). The UDVA was 20/25 or better in 89% of eyes in the spherical IOL group and 93% in the toric IOL group. The DCIVA was 20/32 or better in 92% of eyes in the spherical IOL group and 93% in the toric IOL group at 80 cm (Radner Vissum chart), and 20/32 or better in 100% of eyes in both groups at 63 cm (Colenbrander chart). The DCNVA (Radner chart) was 20/32 or better in 89% of eyes in the spherical IOL group and 90% of eyes in the toric IOL group. There was no difference between the groups in contrast sensitivity, defocus curves, cylinder, or satisfaction results. Conclusion: Patients had significant improvement in visual acuity and gained functional uncorrected visual acuity across all distances in both groups. Satisfaction was high with both IOLs.

    Estabilidad rotacional y caracterización visual de lentes tóricas intraoculares monofocales y multifocales

    Evaluar la estabilidad rotacional y su influencia en la agudeza visual postoperatoria de diferentes lentes intraoculares tóricas monofocales y multifocales. Crear un Sistema de marcado que permita implantar la lente tórica en el eje correcto en el acto quirúrgico. Material y método: Se realizó un estudio prospectivo intervencional. Se incluyeron noventa y uno pacientes con una edad media de 71.65±11.82 años a los que se les implantó una lente intraocular tórica LIOs) tras facoemulsificación. Se implantaron tres LIOs monofocales tóricas (Lentis LT, EnVista toric and Acrysoft IQ toric) y una lente multifocal tórica (Acrysoft IQ Restor). Se tomaron imágenes preoperatorias y postoperatorias para calcular el desalineamiento (misalignment) debido al sistema de marcaje. Para evaluar la rotación en las distintas visitas postoperatorias se tomaron fotografías a la hora, al día, a los 7 días y a los 30 días tras la intervención quirúrgica. La refracción, la mejor agudeza visual sin refracción (UDVA) y la mejor agudeza visual corregida (CDVA) se midieron en la visita postoperatoria al mes de la cirugía...

    Comparing surgically induced astigmatism calculated by means of simulated keratometry versus total corneal refractive power

    Purpose: To evaluate surgically induced astigmatism as computed by means of either simulated keratometry (KSIM) or total corneal refractive power (TCRP) after temporal incisions. Methods: Prospective observational study including 36 right eyes undergoing cataract surgery. Astigmatism was measured preoperatively during the 3-month follow-up period using Pentacam. Surgically induced astigmatism was computed considering anterior corneal surface astigmatism at 3mm with KSIM and considering both corneal surfaces with TCRP from 1 to 8mm (TCRP3 for 3mm). The eyes under study were divided into two balanced groups: LOW with KSIM astigmatism <0.90D and HIGH with KSIM astigmatism ≥0.90D. Resulting surgically induced astigmatism values were compared across groups and measuring techniques by means of flattening, steepening, and torque analysis. Results: Mean surgically induced astigmatism was higher in the HIGH group (0.31D @ 102°) than in the LOW group (0.04 D @ 16°). The temporal incision resulted in a steepening in the HIGH group of 0.15 D @ 90°, as estimated with KSIM, versus 0.28 D @ 90° with TCRP3, but no significant differences were found for the steepening in the LOW group or for the torque in either group. Differences between KSIM- and TCRP3-based surgically induced astigmatism values were negligible in LOW group. Conclusion: Surgically induced astigmatism was considerably higher in the high-astigmatism group and its value was underestimated with the KSIM approach. Eyes having low astigmatism should not be included for computing the surgically induced astigmatism because steepening would be underestimated.
