    Densitometría ósea de los maxilares y del espacio periimplantario

    Este trabajo pretende analizar la densidad mineral ósea de los maxilares edéntulos in vivo mediante el uso de la absorciometría dual de doble energía de rayos X (DEXA). Con ello se quiere valorar las diferencias entre las distintas zonas de los maxilares, así como las diferencias según edad y sexo, elaborar un mapa de densidades y valorar la cicatrización ósea periimplantaria. La primera parte del estudio se realizó en 40 pacientes con zonas edéntulas de más de 9 meses de evolución realizándoseles una densitometría. Entre los resultados destacan una media de densidad general de 0,914 gr/cm2 (s = 0,195) y unas diferencias estadísticamente significativas entre las zonas de los maxilares superior-inferior (p<0,01), anterior-posterior (p<0,05), anteroinferior-posteroinferior (p<0,01), anteroinferior-anterosuperior (p<0,01), inferior derecho-superior derecho (p<0,01), inferior izquierdo-superior izquierdo (p<0,05). No aparecieron diferencias significativas en cuanto al sexo ni a la edad. Se ha realizado un mapa preliminar de densidades según áreas de los maxilares. En el siguiente apartado se realizó un estudio de las medidas de densidad ósea periimplantaria recogidas en el tejido óseo adyacente a los 69 implantes dentales osteointegrados colocados en los pacientes con tres medidas; la ya referida previa a la cirugía, al mes y a los cuatro meses. Los resultados confirmaron un proceso de desmineralización postquirúrgica al mes y remineralización hasta valores superiores a los iniciales al cabo de los cuatro meses. Estos valores finales no presentaron diferencias estadísticamente significativas en las comparaciones entre sexos, en las realizadas entre maxilar y mandíbula, ni por grupos de edad. Con todo ello se comprobó que el DEXA es un método válido para la medida de densidad mineral ósea in vivo de los maxilares, aunque requiere adaptaciones en el dispositivo y el método para su utilización práctica.

    Management of Schneiderian membrane perforations during maxillary sinus floor augmentation with lateral approach in relation to subsequent implant survival rates: a systematic review and metaanalysis

    Background: This systematic review aimed to propose a treatment protocol for repairing intraoperative perforation of the Schneiderian membrane during maxillary sinus floor augmentation (MSFA) procedures with lateral window technique. In turn, to assess subsequent implant survival rates placed below repaired membranes compared with intact membranes and therefore determine whether membrane perforation constitutes a risk factor for implant survival. Material and methods: This review was conducted according to PRISMA guidelines. Two independent reviewers conducted an electronic search for articles published between 2008 and April 30, 2020, in four databases: (1) The National Library of Medicine (MEDLINE/PubMed) via Ovid; (2) Web of Science (WOS); (3) SCOPUS; and (4) Cochrane Central Register of Controlled Trials (CENTRAL); also, a complementary handsearch was carried out. The NewcastleOttawa Quality Assessment Scale was used to assess the quality of evidence in the studies reviewed. Results: Seven articles fulfilled the inclusion criteria and were analyzed. A total of 1598 sinus lift surgeries were included, allowing the placement of 3604 implants. A total of 1115 implants were placed under previously perforated and repaired membranes, obtaining a survival rate of 97.68%, while 2495 implants were placed below sinus membranes that were not damaged during surgery, obtaining a survival rate of 98.88%. The rate of Schneiderian membrane perforation shown in the systematic review was 30.6%. In the articles reviewed, the most widely used technique for repairing perforated membranes was collagen membrane repair. Conclusions: Schneiderian membrane perforation during MFSA procedures with lateral approach is not a risk factor for dental implant survival (p=0.229; RR 0.977; 95% CI 0.941-1.015). The knowledge of the exact size of the membrane perforation is essential for deciding on the right treatment plan.

    Autogenous Dentin Graft in Bone Defects after Lower Third Molar Extraction: A Split-Mouth Clinical Trial

    Various biomaterials are currently used for bone regeneration, with autogenous bone being considered the gold standard material because of its osteogenic, osteoconductive, and osteoinductive properties. In recent years, the use of autogenous dentin as a graft material has been described. This split-mouth clinical trial assesses the efficacy of autogenous dentin for the regeneration of periodontal defects caused by bone loss associated with impacted lower third molar extraction. Fifteen patients underwent bilateral extraction surgery (30 third molars) using dentin as a graft material on the test side, and leaving the control side to heal spontaneously, comparing the evolution of the defects by evaluating probing depth at three and six months post-operatively. Bone density and alveolar bone crest maintenance were also evaluated six months after surgery, and pain, inflammation, mouth opening capacity on the second and seventh days after surgery. Probing depth, radiographic bone density, and alveolar bone crest maintenance showed significant differences between the test and control sides. Autogenous dentin was found to be an effective biomaterial for bone regeneration after impacted lower third molar extraction.

    Proyecto para la autoevaluación del alumno de odontología en conocimientos de cirugía maxilofacial

    En el programa de estudios actual, resulta controvertido el sistema de evaluación del alumno. La enseñanza de Cirugía Bucal comprende la impartición de clases magistrales en las que se exponen los conocimientos que el alumno debe adquirir, seminarios en los que se actualizan los conocimientos científicos actuales, prácticas en las que el alumno adquiere la habilidad manual para desarrollar en un futuro su actividad, y trabajos de revisión o búsqueda bibliográfica en los que el alumno debe aprender a seleccionar la información que le resulte de utilidad, organizarla y exponerla. La asistencia y participación de forma activa en el conjunto de estas actividades programadas debe permitir al alumno adquirir los conocimientos y competencias necesarias para realizar su actividad profesional de forma razonada, responsable y eficaz, siendo consciente mediante una evaluación interna de aquellos aspectos para los que no se encuentra capacitado en base a su experiencia. Por otra parte, resulta complicado evaluar de forma externa por parte del profesorado dicha adquisición cuando una calificación final implica la aptitud o no de forma general del alumno, sin considerar aspectos para los que podría no estar preparado y no ser consciente. En este contexto en el que la evaluación del alumno resulta una responsabilidad exclusiva del profesorado, se pretende hacer consciente al alumno de la importancia de la adquisición de conocimientos por encima del aprobado o suspenso en los conocimientos de una materia. Se propone implicar al alumno en la adquisición de conocimientos, haciendo que sea consciente de su aprendizaje y la importancia que tiene de cara a un futuro profesional en el que sus conocimientos y habilidades influyen directamente en la salud de los pacientes. Para ello, se propone la elaboración y difusión de un libro de cirugía maxilofacial, que suponga un conocimiento más amplio y actualizado del territorio maxilofacial. Este libro pretende ser una guía clínica de referencia para el alumno. La Cirugía Maxilofacial en Europa y Estados Unidos es desde hace años patrimonio del odontólogo. Sin embargo, aunque en España las competencias de la Cirugía Bucal son menos extensas, si resulta de gran importancia que el alumno y el futuro profesional sean capaces de diagnosticar patologías íntimamente relacionadas con su área de conocimiento para favorecer un flujo del sistema de atención sanitaria. Por otra parte, el alumno debe ser consciente en aquellos casos en los que no se encuentre preparado para desempeñar la actividad profesional por la gran responsabilidad que supone.

    Los cambios en la docencia debido a la pandemia por la COVID-19 ha llevado a la reducción de la presencialidad y a un aumento de la docencia online. Por ello se diseñó este proyecto, cuya finalidad fue la adaptación a la docencia online de las prácticas preclínicas de Cirugía Bucal I. Para el desarrollo de este trabajo se realizaron las rúbricas de evaluación de cada módulo de la asignatura, se elaboraron videos y documentación para subir al campus virtual antes de la realización de la práctica. Finalmente, se elaboraron cuestionarios de evaluación de la satisfacción de los estudiantes y profesorado con esta metodología. Los resultados mostraron una elevada satisfacción de ambos grupos, considerándola una herramienta de utilidad en el aprendizaje y de implementación en cursos posteriores.

    Clinical performance of tooth root blocks for alveolar ridge reconstruction

    This systematic literature review set out to investigate the clinical outcomes of autogenous tooth root blocks used for ridge augmentation: survival rates, block resorption, implant survival, post-surgical complications, and histology findings. This review followed PRISMA guidelines. An automated search was made in four databases, supplemented by a manual search for relevant articles published before December 2020. The quality of evidence provided was assessed with the Newcastle-Ottawa Quality Assessment Scale and the Joanna Briggs Institute Critical Appraisal tool. Seven articles fulfilled the inclusion criteria and underwent analysis. The articles included a total of 136 patients, who received 118 autogenous tooth root blocks and 26 autogenous bone blocks showing block survival rates of 99.15% and 100%, respectively. Tooth root blocks presented a mean bone gain that was similar to autologous bone blocks but showed less resorption. The implant survival rate was 98.32% for autogenous tooth root blocks. Reconstruction of alveolar crests by means of autogenous tooth root blocks appears to be a satisfactory option for single-tooth gaps and low grades of bone atrophy in terms of the survival of the bone block and the implants placed subsequently. More research providing long-term data is needed to confirm these findings.

    Split bone block technique: 4-month results of a randomised clinical trial comparing clinical and radiographic outcomes between autogenous and xenogeneic cortical plates

    Purpose: To evaluate short-term clinical and radiographic outcomes of bone regeneration procedures using thin cortical porcine xenogeneic bone plates in combination with autogenous bone chips compared with thin autogenous cortical plates and autogenous bone chips. Materials and methods: A total of 19 patients (12 women and 7 men, mean age 58.24 ± 3.09 years) were randomly allocated to two different groups regarding surgical procedure: autogenous cortical plates (ACP group) and xenogeneic cortical plates (XCP group). Preoperative CBCT scans were performed for each patient. Surgical time and postoperative pain were recorded, as well as tissue healing and graft resorption after 4 months, then another surgical procedure was performed to place dental implants. Data were analysed using an analysis of covariance. Results: Twenty-one surgical procedures were performed on 19 patients (10 from the XCP group and 9 from the ACP group). The operative time was significantly lower in the XCP group (25.45 ± 3.88 minutes) than in the ACP group (44.10 ± 3.60 minutes). The XCP group also showed less pain, but not significantly less, than the ACP group. The graft resorption rate in the ACP and XCP groups was 2.03 ± 1.58% and 3.49 ± 2.38% respectively, showing no statistically significant difference. Conclusions: Despite the limited sample size and non-uniform distribution between the maxilla and mandible as surgical sites, the results suggest that XCP and ACP grafts are similar in terms of bone volume gain and graft resorption rate, with no significant differences in wound healing or complication rate. Nevertheless, the XCP group recorded lower pain levels and required significantly less operative time compared to the ACP group.

    The osteocyte: A multifunctional cell within the bone

    The knowledge of bone biology has largely changed in the last few decades. Osteocytes are multifunc- tional bone cells that are surrounded by mineralized bone matrix and for decades it was considered that they might be relatively inactive cells. However, nowadays it is known that osteocytes are highly active cells which are indispensable for the normal function of the skeleton, playing main roles in several physiological processes, both within and beyond the bone microenvironment. This review highlights and updates the current state of knowledge of the osteocyte and focuses on its roles in bone remodeling and mineral homeostasis, and also reviews its recently discovered endocrine function. Osteocytes secrete sclerostin (a protein that works as a negative regulator of bone mass), and FGF-23, the most important osteocyte-secreted endocrine factor, since it is able to regulate the phosphate metabolism. Moreover, osteocytes can act as mechanosensory cells, transforming the mechanical strain into chemical signal- ing towards the effector cells (osteoblasts and osteoclasts). Therefore, the osteocyte plays an important role in bone biology, specifically in the remodeling process, since it regulates both the osteoblast and osteoclast activity. Finally, the paper discusses the clinical application of the bone biology, updating the new therapies against bone-loss disorders.

    Analysis of Resorption and Need for Overcorrection in Alveolar Distraction

    Purpose: The aim of this study was to evaluate the degree of resorption that occurs following a period of consolidation in alveolar distraction osteogenesis and assess the amount of overcorrection necessary to achieve the planned height. Materials and methods: A retrospective observational study was conducted comprising 19 surgically treated patients, where a total of 19 extraosseous distractors were placed. All patients undergoing surgery had a panoramic radiograph and a computed tomography scan, both at the end of distraction and at the end of the consolidation period. To assess the degree of resorption, the distance from the mandibular border, maxillary sinus floor, or nasal floor to the limit of the crestal bone following consolidation was compared with the height recorded at the end of the distraction period. Results: Resorption occurred in all cases ranging from 7% to 25%. Conclusion: The presence of resorption reported from this study indicates that an overcorrection of at least 20% may be necessary when distraction osteogenesis is performed.
