Abstract
An adverse event (AE) is an unintended harm to a patient as a result of medical care, not directly related to an underlying disease, and that can include prolonged hospitalisation, physical disability at discharge, readmission, risk of death, or death. Knowledge of AEs that arise in acute care hospitals is a crucial aspect of patient safety. Studies found a rate of AEs ranging from 3% to 17%, of which 50% were considered avoidable. Most AEs cause a mild or moderate harm to the patient; however, it has been estimated that a non-negligible percentage cause serious harm and that approximately 7%result in the patient's death. AEs not only harm the patient, but also increase the use of resources and, consequently, the health system expenditure; the AEs that result in the highest expenditure are those related to surgery, medication, and delay in diagnosis...
