Abstract
El presente proyecto de tesis doctoral se titula “Impacto clínico de las infecciones por Enterobacterales productores de carbapenemasas”. Las infecciones por Enterobacterias productores de carbapenemasas (EPC) son un problema de salud emergente, con una elevada morbi-mortalidad asociada y gran capacidad de diseminación. La mayor parte de los estudios publicados hasta este momento han sido diseñados para evaluar los factores de riesgo para el desarrollo de este tipo de infecciones o para establecer el tipo de tratamiento más adecuado. De igual forma, en la mayor parte de los estudios que se centran en los factores pronósticos, el emparejamiento entre casos y controles se realizó en base exclusivamente a la presencia de bacteriemia asociada, y además incluyeron episodios de distintos focos (o incluso no se llega a precisar esta variable). Además, en muchos casos los mecanismos de resistencia son heterogéneos o no están bien especificados, y los centrados específicamente en infecciones por EPC se limitan únicamente a cepas productoras de KPC. Por otro lado, los pacientes con infecciones por EPC presentan mayor carga de comorbilidad y de gravedad en sus enfermedades subyacentes respecto a los pacientes con infecciones por Enterobacterales sensibles a carbapenémicos...
The current doctoral thesis project is titled 'Clinical Impact of infections due to Carbapenemase-Producing Enterobacterales'. Infections due to carbapenemase-producing Enterobacteriaceae (CPE) are an emerging health problem, with a high morbidity and mortality risk and a great spread capacity. Most of the previously published studies have been designed to assess either the risk factors for the development of this type of infection or to determine the most appropriate therapy. Similarly, in most of the studies focused on prognostic factors, the matching criteria between cases and controls was limited to the concurrent presence of bacteremia and included infections from different sources (or did not even specify such a variable). In addition, in many of these studies the underlying resistance mechanisms were heterogeneous or not well characterized, whereas those performed specifically on CPE infections were limited only to KPC-producers. On the other hand, it is worth noting that patients with CPE infections have a higher comorbidity burden and severity of underlying conditions compared to those with infections due to carbapenem-susceptible strains...
