Abstract
La determinación plasmática de la proadrenomedulina de región media (MR-proADM) ha demostrado ser un biomarcador pronóstico útil en patologías infecciosas graves en población adulta. Sin embargo, apenas existen estudios que analicen su precisión como marcador de afectación parenquimatosa renal en la infección urinaria en pediatría. Un biomarcador preciso podría detectar precozmente a los pacientes con daño renal, evitando otras pruebas invasivas como la gammagrafía renal.El objetivo de este estudio es valorar la utilidad de la MR-proADM como biomarcador de afectación parenquimatosa aguda y crónica en la infección del tracto urinario (ITU) febril en población pediátrica. Material y métodos. Se ha realizado un estudio de cohorte en pacientes pediátricos con diagnóstico de ITU febril que acudieron a un servicio de urgencias pediátricas hospitalario entre enero de 2015 y diciembre de 2018. Se determinaron los niveles de MR-proADM en plasma y orina, además de otros parámetros de laboratorio (proteína C reactiva, procalcitonina, bioquímica urinaria, entre otros). Tras confirmación de la ITU febril se realizó gammagrafía en fase aguda para objetivar afectación parenquimatosa renal, y en fase de seguimiento pasados 9 meses para determinar cicatrices renales. Plasmatic midregional-proadrenomedullin (MR-proADM) is a useful prognostic peptide in severe infectious pathologies in the adult population. However, there are no studies that analyse its accuracy in parenchymal damage in febrile urinary tract infection (UTI) in children. An accurate biomarker would provide an early detection of patients with kidney damage, avoiding other invasive tests like renal scintigraphy scans. The objective of this study is to evaluate the usefulness of MR-proADM as a biomarker of acute and chronic renal parenchymal damage in febrile UTI within the paediatric population. Methods. A prospective cohort study was conducted in paediatric patients with febrile UTI attended at a paediatric emergency department between January 2015 and December 2018. Plasma and urine MR-proADM levels were measured at admission in addition to other laboratory parameters (C-reactive protein, procalcitonin, amongst others). After confirmation of febrile UTI, renal scintigraphy scans were performed during the acute and follow-up stages to detect pyelonephritis and renal scars, respectively.
