Abstract
Tras el implante de una lente fáquica de cámara posterior (pIOL), como la lente EVO ICL™ (STAAR Surgical Inc, Monrovia, CA, USA)., el valor del vault, distancia existente entre la cara posterior de la pIOL y la cara anterior del cristalino, ha sido considerado como la variable más importante para valorar el correcto posicionamiento y sizing de la misma. Confinada entre valores de 250μm y 750μm, cualquier valor de vault que estuviera fuera de ese rango era considerado incorrecto. Los nuevos modelos de lentes EVO ICL™ junto con nuevas líneas de investigación y evidencia científica nos indican que el vault es una medida dependiente de otras variables anatómicas, como la curvatura del cristalino, la dinámica pupilar, la iluminación o la acomodación. Estas impactarán directamente sobre su valor. A esto hay que sumar el papel de los ángulos iridocorneales tras el implante de la lente EVO ICL™. Posibles variaciones de estas estructuras podrían causar alteraciones anatómicas o problemas secundarios a la cirugía. Esta situación hace que el vault sea una medida variable y débil a la hora de definir el correcto posicionamiento de la lente. After implantation of a posterior chamber phakic lens (pIOL), such as the EVOICL™ lens (STAAR Surgical Inc, Monrovia, CA, USA)., the vault value, known as the distance between the posterior surface of the pIOL and the anterior surface of the crystalline lens, has been considered the most important variable to assess the correct positioning and sizing of the lens. Confined to values between 250μm and 750μm, any vault value outside this range was considered incorrect. The new EVO ICL™ lens models together with new lines of research and scientific evidence indicate that vault is a measurement dependent on other anatomical variables, such as lens curvature, pupillary dynamics, and illumination. These will have a direct impact on its value. To this must be added the role of the iridocorneal angles after implantation of the EVOICL™ lens. Possible variations of these structures could cause anatomical alterations or problems secondary to surgery. This situation makes vault a variable and weak measure when it comes to defining the correct positioning of the lens. It is therefore necessary to define another measure that can give a more independent indication of the correct positioning of the lens...
