Abstract
La candidiasis, ya sea superficial o invasora, representa un problema de salud a nivel global, y plantea grandes retos clínicos debido a la dificultad para diferenciar colonización de infección por Candida, al aumento de la resistencia antifúngica, y a la limitada efectividad de los tratamientos actuales frente a cepas multirresistentes. En este contexto, el desarrollo de herramientas moleculares para estudiar la epidemiología y los mecanismos de resistencia de Candida spp es esencial. Estas herramientas, no sólo permiten identificar clones circulantes en entornos hospitalarios, sino también la posibilidad de establecer la relación entre cepas colonizadoras e invasoras, un aspecto crítico para diseñar estrategias preventivas y de manejo de los pacientes. Asimismo, la caracterización molecular de cepas resistentes contribuye a comprender cómo surgen y se diseminan los clones multirresistentes, lo cual es fundamental para implementar medidas de control de infección hospitalaria, reducir la transmisión, y, por lo tanto, disminuir así la probabilidad de aparición de brotes nosocomiales. Este último aspecto, se ilustra bien con el reciente aumento global de brotes causados por C. parapsilosis resistente a fluconazol, sobre todo en el sur de Europa...
Both, superficial and invasive candidiasis, pose a global health threat and raise huge clinical challenges due to the difficulty in differentiating Candida colonisation from infection, increasing antifungal resistance, and limited efficacy of current antifungal agents against multidrug-resistant strains. In this scenario, the development of molecular tools to study the molecular epidemiology and resistance mechanisms of Candida spp. have become a must. Those tools do not only allow the detection of circulating clones across the clinical setting but also settle bonds between colonising and invasive strains, which is crucial for the implementation of preventive strategies and patient management. Moreover, molecular characterisation of resistant strains contributes to a better understanding of multi-resistant clones emergence and spread, which is key to abate patient-to-patient transmission and hinder the emergence of nosocomial outbreaks. That is well illustrated by the recent global increase in the number of outbreaks caused by fluconazole-resistant C. parapsilosis, especially in the south of Europe...
