Abstract
Un importante porcentaje de los errores de medicación se produce durante las transiciones asistenciales, pudiendo afectar tanto a la efectividad de los tratamientos como a la seguridad del paciente. El objetivo de la conciliación de la medicación es garantizar que el paciente recibe los medicamentos que estaba tomando de forma previa a una transición asistencial, en la dosis y pauta correctas y adecuándose a su nueva situación clínica, consiguiendo así disminuir los errores. Existe gran evidencia a favor de la conciliación de la medicación como estrategia para reducir los errores en población adulta, pero son escasos los estudios realizados en pediatría...
