Rapid Detection of Staphylococcus aureus in Lower Respiratory Tract Secretions from Patients with Suspected Ventilator-Associated Pneumonia: Evaluation of the Cepheid Xpert MRSA/SA SSTI Assay

dc.description.abstractA preclinical evaluation was conducted to evaluate the performance of the Cepheid Xpert assay on 135 lower respiratory tract secretions for detection of methicillin-resistant Staphylococcus aureus and S. aureus. Compared with the quantitative culture, the sensitivity, specificity, and positive and negative predictive values were 99.0%, 72.2%, 90.7%, and 96.3%, respectively.
dc.description.abstractEste estudio evalúa el rendimiento del ensayo molecular Xpert MRSA/SA SSTI para la detección rápida de Staphylococcus aureus (MSSA) y Staphylococcus aureus resistente a meticilina (MRSA) directamente en aspirados traqueales de pacientes con sospecha de neumonía asociada a ventilación mecánica (VAP). Se analizaron 135 muestras. Comparado con el cultivo cuantitativo (gold standard), el ensayo mostró: Sensibilidad: 99% Especificidad: 72,2% VPP: 90,7% VPN: 96,3% El test identificó correctamente casi todos los MRSA/MSSA incluso en muestras con bajo inóculo y proporcionó resultados en menos de 1 hora. Se observaron algunos resultados positivos en cultivos negativos, atribuibles a ADN residual o tratamiento antibiótico previo. El ensayo falló en un único MRSA por no detectar el elemento SCCmec. Los autores concluyen que Xpert MRSA/SA SSTI es rápido, sensible y útil para apoyar decisiones terapéuticas tempranas en VAP.
