Efecto de los posteos mediales de retropié sobre la posición de calcáneo y tibia en estática

Abstract

Fundamento y objetivo.

Existe una gran variedad etiológica para la patología del pie y miembro inferior, siendo una de ellas la mala alineación de los segmentos que lo conforman. Las ortesis plantares son un elemento terapéutico ampliamente utilizado. Su efecto clínico de disminución de la sintomatología dolorosa está demostrado, pero su mecanismo de acción aún no lo está. El objetivo del presente estudio es comprobar si existen movimientos en la alineación de la tibia y el calcáneo relacionados con el uso aislado de dispositivos de retropié con distintas alturas a nivel medial en estática.

Material y método.

Se seleccionaron 17 sujetos sanos para la realización del presente estudio y se midió en bipedestación la posición de la tibia y el calcáneo en los tres planos del espacio en 4 situaciones diferentes: sin dispositivo ortésico de retropié y con un dispositivo de retropié de 3, 6 y 9 mm de altura a nivel medial. Para las mediciones se utilizó el goniómetro electrónico Fastrak Polhemus Patriot ®.

Resultados.

El uso aislado de dispositivos de retropié con distintas alturas a nivel medial provocó en el calcáneo un movimiento de inversión (adducción, varo y flexión plantar) y en la tibia movimientos en rotación interna, valgo y desplazamiento posterior de forma significativa (p<0,05) entre las distintas situaciones.

Conclusiones.

El uso aislado de dispositivos de retropié con diferentes alturas a nivel medial influye en la posición de tibia y calcáneo en los tres planos del espacio.

Grounds and objetive.

There is an etiology variety for the foot pathology and the lower extremity, being one of them the bad alignment of its sectors. The plantar orthoses are an therapeutic element that are commonly used. Its clinical effect of the reduction of painful symptomatology is demonstrated, but its mechanism of action is still to be proved. This study’s objective is to confirm if there are movements in the tibial and calcaneus alignment related to the isolated use of rearfoot orthotic device with different medial heights in static.

Material and methods.

17 healthy subjects were selected for this study and the tibial and calcaneus position was measured, while the subjects were standing up, in the three planes of space in 4 different situations: without orthotic device and with rearfoot orthotic device of 3, 6 and 9 mm medial height. The electronic goniometry Fastrak Polhemus Patriot ® was used for the measurement.

Results.

The isolated use of rearfoot orthotic devices with different medial heights produced significant inversion movement in the calcaneus (adduction, varus and plantar flexion) and an internal rotation, valgus and rear movement in the tibial (p<0,05) during the different situations.

Conclusions.

The isolated use of rearfoot orthotic devices with different medial heights affects the tibial and calcaneus posistion in the three planes of space.
