Inmunología de la enfermedad inflamatoria intestinal

Abstract

Bajo el concepto de Enfermedad inflamatoria intestinal se incluye un grupo de enfermedades de etiología desconocida que se caracterizan por la inflamación crónica del intestino debido a procesos patogénicos autoinmunes, las principales presentaciones son la enfermedad de Crohn y la colitis ulcerosa. Además de la influencia de los factores ambientales y la predisposición genética de los enfermos, se observa una mucosa intestinal con respuesta inmune alterada frente a antígenos de la dieta y de la propia microflora, activándose múltiples mecanismos mediados por diversas células y sustancias que conllevan daño tisular inflamatorio, y con su posterior mantenimiento y progresión, agravamiento y cronicidad. Los mecanismos involucrados en estos procesos son muy complejos y todavía no del todo bien conocidos. Se han identificado defectos tanto en la inmunidad innata como en la adaptativa así como en los sistemas reguladores encargados de contrarrestar la inflamación. El estudio de los componentes de la inmunidad innata y adaptativa involucrados en la patogenia abrirá nuevos horizontes en el conocimiento sobre los mecanismos inmunológicos implicados en la inflamación intestinal pudiendo acercarnos hacia una mayor comprensión y posibilitando el desarrollo de estrategias terapéuticas con mayor eficacia que las actuales.

Under the concept of Inflammatory bowel disease are included a group of diseases of unknown etiology that are characterized by chronic inflammation of the intestine due to autoimmune pathogenic processes, the main presentations are Crohn’s disease and ulcerative colitis. In addition to the influence of environmental factors and the genetic predisposition of the patients, an intestinal mucosa is observed with an altered immune response to antigens of the diet and to the own microflora, activating multiple mechanisms mediated by various cells and substances that cause inflammatory tissue damage, and with its subsequent maintenance and progression, aggravation and chronicity. The mechanisms involved in these processes are very complex and are not well known yet. Defects have been identified in both innate and adaptative immunity as well as in the regulatory systems, responsible for counteracting the inflammation. The study of the components of the innate and adaptive immunity involved in the pathogenesis, will open new horizons in the knowledge about the immunological mechanisms involved in the intestinal inflammation, being able to approach towards a greater understanding and enabling the development of therapeutic strategies with greater effectiveness than the current ones.
