Análisis tridimensional de los cambios producios en el maxilar superior con el uso de disyuntor Hyrax y MARPE en pacientes sin crecimiento finalizado.

Abstract

Este estudio buscó evaluar los cambios bi y tridimensionales, dentarios y en la arcada, producidos en el maxilar durante la disyunción maxilar con los dispositivos Hyrax y MARPE-MSE II en pacientes con crecimiento. Los pacientes que cumplieron con los criterios de inclusión y exclusión fueron divididos en dos grupos según su índice de maduración ósea y su valor en la gráfica de curva de crecimiento. El grupo Hyrax fue integrado por 4 pacientes (edad media 10,57 ± 0,86) y recibió tratamiento con disyuntor tooth-borne de 4 bandas y tornillo medio Hyrax. El grupo MARPE tuvo 7 pacientes (edad media 15,24 ± 1,82) y recibió tratamiento mediante disyuntor MSE II. Los cambios ocurridos en el maxilar se evaluaron sobre modelos digitales obtenidos previo al tratamiento y pasados 3 meses desde la última activación. En ambos grupos se observó un aumento en la anchura maxilar anterior y posterior (p0,05), y en el grupo MARPE esta variable no mostró diferencias. El área y el volumen palatino aumentaron en ambos grupos, pero solo fue estadísticamente significativo en el grupo MARPE. En el grupo Hyrax cada milímetro de apertura del tornillo generó un aumento de 72,55 mm2 del área y 181,73 mm3 del volumen palatino, mientras que en el grupo MARPE estos valores fueron de 52,94 mm2 y 138,89 mm3 . No se observaron diferencias estadísticamente significativas entre los grupos. Ambos dispositivos logran adecuada disyunción maxilar. La ausencia de grandes diferencias entre los dispositivos, indica que el uso de disyuntor Hyrax es apropiado en pacientes con estadios tempranos de maduración ósea, y que el uso de disyuntor MARPE MSE II es apropiado en pacientes con estadios de maduración un poco más avanzados.

This study aimed to evaluate two- and three-dimensional tooth and arch changes in the maxilla during maxillary disjunction with the Hyrax and MARPE-MSE II devices in growing patients. Patients who met the inclusion and exclusion criteria were divided into two groups according to theirskeletal maturation index and their growth curve values. The Hyrax group consisted of 4 patients (mean age 10,57 ± 0,86) who received treatment with a 4-banded Hyrax tooth-borne expander. The MARPE group consisted of 7 patients (mean age 15.24 ± 1.82) who were treated with MSE-II. Maxillary changes were evaluated on digital models obtained before treatment and after 3 months since its last activation. Both groups showed an increase in anterior and posterior maxillary width (p0.05), yet this variable was unchanged in the MARPE group. Palatal area and volume increased in both groups, yet it was only statistically significant in the MARPE group. In the Hyrax group each millimeter of screw expansion generated a maxillary area increase of 72,55 mm2 and a maxillary volume increase of 181,73 mm3 , while in the MARPE group these values were 52,94 mm2 and 138,89 mm3 . No statistically significant differences were observed between the groups. Both devices achieve adequate maxillary disjunction. The absence of significant differences between the devices indicates that the use of the Hyrax expander is appropriate in patients with early stages of skeletal maturation, and that the use of the MARPE-MSE II expander is appropriate in patients with slightly more advanced stages of skeletal maturation.
