Estudio de la efectividad de la vacuna profiláctica frente al virus del papiloma humano en la prevención de recurrencia en mujeres que han recibido una terapia escisional por HSIL/CIN : estudio Venus

Abstract

La infección por el virus del papiloma humano (VPH) es la principal causa de lesiones intraepiteliales cervicales y cáncer de cérvix. La vacunación profiláctica frente al VPH es la herramienta más eficaz para la prevención primaria de la infección y las lesiones asociadas. Las pacientes conizadas por HSIL/CIN presentan un riesgo incrementado de desarrollar recurrencia y cáncer de cuello de útero. Aunque estudios sugieren que la vacunación profiláctica frente al VPH podría reducir este riesgo, aún existe controversia. Además, persisten interrogantes sobre su eficacia en casos con infección persistente y el momento óptimo de administración. La Comunidad de Madrid fue pionera en financiar la vacunación profiláctica frente a VPH en el calendario del adulto, incluyendo desde abril de 2015 a mujeres menores de 45 años con diagnóstico de HSIL/CIN 2+ tratadas en los tres años previos. La Unidad de Patología del Tracto Genital Inferior del Hospital Clínico San Carlos fue uno de los primeros centros en aplicar esta medida. Human papillomavirus (HPV) infection is the primary cause of cervical intraepithelial lesions and cervical cancer. Among preventive strategies, prophylactic HPV vaccination is the most effective approach for preventing both the infection and related lesions. Conized women with HSIL/CIN face a higher likelihood of lesion recurrence and subsequent cervical cancer development. Current scientific evidence shows that prophylactic HPV vaccination in women who have been treated for HSIL/CIN is associated with a reduced risk of new lesion development. However, the benefit of vaccination in terms of lesion recurrence remains a topic of ongoing debate. Additionally, important questions persist regarding its efficacy in cases of persistent infection and the optimal timing for administration. The Community of Madrid was among the first to incorporate funded prophylactic HPV vaccination into the adult immunization schedule, initiating in April 2015 the provision of vaccination for women under 45 years who had been treated for HSIL/CIN 2+ within the preceding three years. The Lower Genital Tract Pathology Unit at Clínico San Carlos Hospital was one of the first centers to implement this measure. Building upon this implementation and the growing scientific evidence, it is hypothesized that prophylactic HPV vaccination in women who have undergone conization may be associated with a reduced risk of recurrence compared to their unvaccinated counterparts.
