Abstract
Acute Coronary Syndrome (ACS) is a severe clinical condition that causes a sudden and significant reduction in blood flow to the heart. Its treatment and management require not only medical intervention but also the active involvement of the patient in their recovery process. Despite advancements in cardiovascular education led by nursing professionals at the Cardiovascular Institute (ICV) of Hospital Clínico San Carlos (HCSC), challenges persist in patient follow-up and autonomy after hospital discharge.Various factors, such as a lack of knowledge about their health status, inadequate treatment adherence, the absence of a structured follow-up plan, and insufficient assessment of emotional and functional status, limit patient’s ability to manage their disease effectively. Patient empowerment emerges as an innovative approach that enables individuals to take control of their health through education, training, and self-efficacy.This study focuses on evaluating the impact of an educational and training intervention on patients with ACS after hospital discharge, with continuous follow-up over one year. The aim is to enhance their empowerment, reduce anxiety, and decrease hospital readmissions and emergency visits, among other factors, promoting positive health changes and reducing the need for hospital care...
