Abstract
La crisis sanitaria ocasionada por el coronavirus del síndrome respiratorio agudo severo de tipo 2 (SARS-CoV-2) ha causado millones de contagios y fallecimientos a nivel global caracterizándose por una gran heterogeneidad clínica. La edad avanzada, el sexo masculino y comorbilidades (obesidad, diabetes, hipertensión) se asociaron a peor pronóstico, pero no explican por completo la variabilidad clínica. Desde el comienzo de la pandemia, la inmunidad innata surgió como un elemento clave en el control de la enfermedad asociándose la producción excesiva de citoquinas proinflamatorias, la hiperactivación de macrófagos y una respuesta deficiente de la vía del interferón tipo I a cuadros más graves. Entre las alteraciones inmunológicas descritas, se encontraba el compartimento de monocitos, esencial en la inmunidad antiviral y en la modulación de la respuesta adaptativa...
The health emergency caused by Severe Acute Respiratory Syndrome Coronavirus 2(SARS-CoV-2) has led to millions of infections and deaths worldwide, characterized bya striking clinical heterogeneity. Advanced age, male sex, and comorbidities (obesity, diabetes, and hypertension) were identified early as factors associated with poorer prognosis; however, these variables alone are insufficient to account for the marked heterogeneity observed in clinical outcomes. From the onset of the pandemic, innate immunity emerged as a key determinant in disease control, with excessive production of proinflammatory cytokines, macrophage hyperactivation, and an impaired type Iinterferon response being linked to severe outcomes. Among the immunological alterations described, particular attention was drawn to the monocyte compartment, which plays a central role in antiviral defense and in shaping adaptive responses...
