Abstract
La esquizofrenia es un trastorno mental crónico caracterizado por recurrencias de los síntomas a lo largo del curso de la enfermedad. Estas recurrencias pueden estar asociadas a un deterioro progresivo. La principal causa de tales recurrencias es la interrupción del tratamiento con fármacos antipsicóticos. Estas interrupciones del tratamiento son muy habituales, y difíciles de cuantificar fiablemente. Esta circunstancia supone que, a día de hoy, no se sepa exactamente en qué medida los pacientes con esquizofrenia tienen recurrencias de sus síntomas incluso cuando se siguen las recomendaciones de mantener el tratamiento. Entender esto es crucial para caracterizar el curso de la enfermedad en condiciones de tratamiento óptimas. Además, entender la epidemiología y biología de la recurrencia de psicosis en distintas circunstancias (por ejemplo, tras interrumpir el tratamiento, o a pesar de continuarlo), es esencial para poder desarrollar instrumentos predictivos del riesgo de recurrencia que puedan ser útiles en la clínica. Schizophrenia is a chronic disorder characterized by relapses throughout the course of illness. These relapses may be associated with progressive deterioration. The main cause of relapses is treatment non-adherence, which is very prevalent but challenging to quantify reliably. Thus, to date it is not well understood the extent to which individuals with schizophrenia relapse despite guaranteed treatment delivery. Understanding this is crucial to characterize the course of the disease under optimal treatment conditions. In addition, understanding the epidemiology and biology of psychosis relapse under different circumstances (for example, after stopping treatment, or despite continuing it), is essential to develop predictive instruments of relapse that can be useful in the clinic. The central theme of this doctoral thesis work is to study the relapse of psychosis in people treated with long-acting injectable antipsychotics, since for these individuals the continuous exposure to medication can be measured exactly...
