Abstract
La obesidad es una enfermedad crónica y multifactorial que constituye actualmente un problema global, siendo uno de los principales desafíos en salud pública. El impacto de la obesidad, además de la morbilidad asociada a las múltiples enfermedades que se asocian, tiene unos costes sanitarios y personales importantes. A pesar de los avances en el tratamiento farmacológico de la obesidad, los resultados a largo plazo son limitados. En este contexto, la cirugía bariátrica ha demostrado ser el tratamiento más efectivo a largo plazo para el tratamiento de la obesidad y las enfermedades asociadas. Entre las técnicas disponibles, el bypass gástrico en Y de Roux (BPGYR) sigue considerándose el procedimiento de referencia. La reganancia de peso (15–41%) a partir del segundo año postoperatorio tras el BPGYR se ha relacionado con la dilatación de la anastomosis gastroyeyunal y del reservorio disminuyendo la restricción a la ingesta y la sensación de saciedad. Para evitarlo se ha propuesto el cerclaje del reservorio gástrico (BPGYR-A), el cual preservaría la restricción a partir de los dos años, independientemente de la dilatación del estoma. Beyond the morbidity associated with the numerous comorbidities it entails, obesity also imposes a substantial personal and health care burden.
