Ligamento anterolateral de la rodilla. ¿realidad o ficción?

Abstract

Recientemente ha sido descubierto un nuevo ligamento en la parte lateral de la rodilla, que une el fémur con la tibia y al que se le ha denominado ligamento anterolateral (LAL) de la rodilla 1. Existen diversas fuentes que aseguran que el LAL ya había sido descrito anteriormente por otros autores. La primera descripción de esta estructura, se realizó en 1879 por un cirujano francés, denominado Paul Segond7, quien la describió como una "banda fibrosa resistente con aspecto nacarado” y la definió como un “refuerzo de la cápsula lateral” por fibras de la cintilla iliotibial (CIT). Así, a lo largo de la historia,numerosos autores fueron refiriéndose a esta estructura con distintos nombres como: “ligamento capsular lateral” 4-5, “ligamento femorotibial anteroexterno”59,“ligamento capsular lateral” en el tercio medio de la cápsula articular lateral 60-61,“banda anterior” del ligamento colateral lateral 3,62, hasta que Vincent y cols66 enel año 2012 le dieron el nombre por el que ahora es conocido, LAL de la rodilla.Si bien es verdad, que no se conoce definitivamente si todos estos nombres se refieren a la misma estructura, sí hay consenso en la presencia de una estructura ligamentosa que se origina en el epicóndilo femoral lateral describiendo un recorrido anterolateral hasta insertarse posterior al tubérculo de Gerdy (TG), en la cara anterolateral proximal de la tibia1,67...

Recently, a ligament located on the lateral aspect of the knee, termedanterolateral ligament (ALL), and which connects the femur to the tibia, hasbeen discovered1. Several works, however, argue that ALL had been previouslydescribed by other authors. The first such description, from 1879, is that of theFrench surgeon Paul Segond7, who reported the structure to be a “pearly,resistant, fibrous band,” defining it as stabilizer of the lateral capsular ligamentby fibers of the iliotibial band (ITB). Since then, numerous researchers havemade reference to this structure under a variety of names, such as: “lateralcapsular ligament”4-5, “anterolateral femorotibial ligament”59, “mid-third lateralcapsular ligament”60-61, and anterior band of the lateral collateral ligament(LCL)3,62, until 2012, when Vincent et al.66 gave it the name it goes by today,that is, ALL of the knee. Though no definitive evidence indicates that theseterms refer to the same structure, there is a consensus regarding the presenceof a ligamentous structure originating in the lateral femoral epicondyle whosecourse runs to the anterolateral tibia and with an insertion posterior to Gerdy'stubercle (GT) on the anterolateral aspect of the proximal tibia1,67...
