Abstract
La enfermedad de Parkinson (EP) es una enfermedad neurodegenerativa producida fundamentalmente por la degeneración progresiva del sistema dopaminérgico estriatal. Los tratamientos de sustitución dopaminérgica son empleados con el fin de reemplazar este déficit. Sin embargo, estas terapias y en especial los agonistas dopaminérgicos (AD) se han relacionado con la aparición de complicaciones neuropsiquiátricas entre las que destacan el trastorno del control de impulsos (TCI) y otros comportamientos compulsivos relacionados. El TCI es una alteración psiquiátrica caracterizada por un incremento de la impulsividad y la incapacidad para resistir una tentación, urgencia o impulso a pesar de que pueda plantear consecuencias negativas para el propio individuo o su entorno. Entre otros factores, una edad precoz de inicio de la EP, una personalidad previa con rasgos impulsivos o los antecedentes de adicciones a sustancias, parecen aumentar el riesgo de desarrollarlo. Las manifestaciones más frecuentes varían dentro del espectro del trastorno de juego, la ingesta compulsiva, la compra compulsiva y la hipersexualidad. Habitualmente también se incluyen en esta clasificación otros comportamientos compulsivos relacionados como el punding, el hobbismo y el síndrome de disregulación dopaminérgica (SDD)...
Parkinson's disease (PD) is a neurodegenerative disease primarily caused by progressive degeneration of the striatal dopaminergic system. Dopaminergic replacement treatments are used in order to replace this deficit. However, these therapies – especially dopamine agonists (DA)– have been related to the appearance of neuropsychiatric complications, including impulse control disorder (ICD) and other related compulsive behaviors. ICD is a psychiatric disorder characterized by increased impulsivity and inability to resist a temptation, urge or impulse, even though it may pose negative consequences for the individuals themselves or their environment. Among other factors, an early age at PD onset, a previous personality with impulsive traits or a history of substance addictions seem to increase the risk of developing ICD. The most frequent clinical manifestations are gambling disorder, compulsive buying, binge-ea|ng disorder and hypersexuality.
