Abstract
La albuminuria se define como la presencia de albúmina en orina debido a la pérdida de selectividad de la barrera de filtración glomerular. Es un marcador temprano de daño renal y vascular asociado al desarrollo de complicaciones cardiovasculares y renales en pacientes con diabetes mellitus e hipertensión. Dichas alteraciones podrían estar relacionadas con un aumento del estrés oxidativo tanto a nivel renal como vascular. La rata Munich Wistar Frömter (MWF) es un modelo genético de albuminuria espontánea e hipertensión moderada que cursa con enfermedad renal crónica por lo que es un modelo interesante para el estudio de las alteraciones vasculares asociadas a la albuminuria, así como para la identificación de nuevas estrategias terapéuticas. La finerenona, potente y selectivo antagonista del receptor mineralocorticoide, ha demostrado, en ensayos clínicos recientes, su eficacia en la reducción de la albuminuria, así como efectos beneficiosos a nivel cardiovascular, constituyendo una estrategia terapéutica muy prometedora en el tratamiento de la enfermedad renal crónica y de la nefropatía diabética. Teniendo en cuenta estos resultados, el desarrollo de un modelo de nefropatía diabética en la rata MWF podría constituir una herramienta muy útil en el desarrollo de futuras estrategias farmacológicas para el manejo de las complicaciones renales y cardiovasculares asociadas a esta patología...
Albuminuria is defined as the presence of albumin in urine due to the loss of selectivity of the glomerular filtration barrier. It is an early marker of renal and vascular damage associated with the development of cardiovascular and renal complications in patients with diabetes mellitus and hypertension. Such alterations could be related to an increase in oxidative stress at both renal and vascular levels. The Munich Wistar Frömter (MWF) rat is a genetic model of spontaneous albuminuria and moderate hypertension that occurs with chronic kidney disease. It is therefore an interesting model for the study of vascular alterations associated with albuminuria, as well as for the identification of new therapeutic strategies. Finerenone, a potent and selective mineralo corticoid receptor antagonist, is effective in reducing albuminuria and has beneficial effects at the cardiovascular level, becoming a promising therapeutic strategy for the treatment of chronic kidney disease and diabetic nephropathy. Considering these results, the development of a model of diabetic nephropathy in the MWF rat might be a very useful tool for the development of future pharmacological strategies for the management of renal and cardiovascular complications associated with this pathology...
