Perspectiva de los nuevos anticoagulantes orales (nacos)

Abstract

Las expectativas generadas por los nuevos anticoagulantes orales en cuanto a eficacia, seguridad y facilidad de administración, abre una nueva etapa en el tratamiento antitrombótico. La adecuada elección entre un anticoagulante tradicional o uno nuevo, plantea numerosos interrogantes a los clínicos, al no existir unanimidad de opiniones entre expertos a nivel mundial. Todavía se deben depurar algunos inconvenientes para que su uso constituya una práctica segura. El propósito de esta revisión bibliográfica es documentar la diferencia de criterios en cuanto a su indicación, los beneficios que reportan al paciente y las barreras que dificultan su plena utilización. A partir de los datos obtenidos en los estudios realizados, se puede afirmar que en estos momentos, existen notables diferencias metodológicas, que junto con la ausencia de estudios comparativos entre estos nuevos fármacos, hace difícil la comparación directa entre ellos para valorar y juzgar la eficacia y seguridad de los mismos.

The expectations generated by the new oral anticoagulants in efficacy, safety and ease of administration, opens a new stage in the antithrombotic treatment. The right choice between a traditional anticoagulant or a new one, raises numerous questions clinicians, as there unanimity of views among experts worldwide. Should still debug some drawbacks to its use constitutes a safe practice. The purpose of this review is to document the disagreement in terms of indication, the benefits derived from the patient and barriers that hinder their full utilization. From the data obtained in the studies, we can say that there are significant methodological differences, together with the absence of studies comparing these new drugs makes it difficult to directly compare between them to assess and judge the effectiveness right now, and safety thereof.
