Abstract
El diagnóstico de la enfermedad celíaca (EC) en pacientes que adoptan una dieta sin gluten (DSG) antes de disponer de un diagnóstico confirmado constituye un reto importante en la práctica clínica. En estos casos, las guías clínicas actuales solo contemplan reintroducir el gluten en la dieta con el objetivo de inducir nuevamente las alteraciones histológicas y serológicas características de la enfermedad. No obstante, esta estrategia presenta importantes limitaciones, entre ellas la escasa tasa de aceptación de los pacientes, debido al temor de reaparición de los síntomas, y a la ausencia de un protocolo estandarizado respecto a la cantidad de gluten a administrar y la duración de la reintroducción. Como consecuencia, en las últimas décadas se han propuesto diversas alternativas diagnósticas para tratar de identificar la EC evitando una reexposición prolongada al gluten, incluyendo estrategias aplicables tanto en pacientes en DSG como en aquellos sometidos a reintroducciones breves y controladas (de uno a tres días)...
The diagnosis of celiac disease (CD) in patients who start a gluten-free diet (GFD) before a confirmed diagnosis remains a major challenge in clinical practice. In such cases, current clinical guidelines recommend gluten reintroduction to reproduce the characteristic histological and serological alterations of the disease. However, this approach has substantial limitations, including low patient acceptance due to the risk of symptom recurrence, and the lack of a standardized protocol regarding both the amount of gluten to be consumed and the duration of reintroduction. Consequently, in recent decades, several alternative diagnostic approaches have been proposed to identify CD while minimizing prolonged gluten exposure, including strategies applicable to patients already on a GFD as well as those undergoing short and controlled gluten reintroductions lasting either 1 or 3 days...
